And from that failure, she became even more displeased, and so on leading her to eventually lose faith in her study. Dillard, annoyed, states, â€Å"The kit’s diatomaceous earth was a bust† (Chaffee 49). After waiting so many years for a microscope to investigate the countless things she had in mind, Dillard slowly built up high expectations in what she would see. When her high hopes were brought to a halt, Dillard’s beliefs about science and its amazement were suddenly brought to question. For example, when she talks about the sample from the â€Å"Cliffs of Dover† she is expecting to see something much more animated and detailed than, well, just a closer picture of a rock. Since she had these high expectations crushed so fast, she couldn’t help but think that somewhere out there waiting for her were all those little  organisms she read so much about. Convinced that science was more than just a larger picture, Dillard finds herself wandering about in a park where she comes across a small puddle that just had to be full of little critters, and in her last desperate attempt to prove to herself that there really are interesting things out there, she takes a sample. From this she finds what she has been looking for all along: her amoeba. After all this doubt and frustration, Dillard earned her prize. She was extraordinarily pleased and felt that everybody must know; everybody must see this amazing little creature rolling awkwardly about within its small drop of water on her slide. Much to her surprise, when she ran upstairs to share this seemingly imperative news with her family, nobody seemed to care. Her parents simply smiled and brushed her aside, continuing on with their after-dinner coffee. This is when Dillard’s belief about her private infatuation with science is strengthened, and she states: â€Å"You do what you do out of your own private passion for the thing itself,† (Chaffee 50) and returned to her private wonder down the stairs. Without her parents playing a major role in her life, Dillard is left to investigate the world up close and personal on her own. Perhaps the sole reason she grew so fond of science is because it had a set of basic rules and guidelines which set a path she never had in her life at home. With science forming a foundation for her to stand on, it most likely brought her comfort in knowing she had something to fall back on. Since her parents rarely gave her any real source of motivation, Dillard is forced to look within herself to keep moving on. Thinking that â€Å"The sky is the limit† and â€Å"Anything was possible† (Chaffee 50) was enough to keep her focused on searching for her dreams. If it weren’t for these high hopes and faith, Dillard may have totally given up her search, and thus never have found her precious amoeba. Perhaps Dillard continued searching for her creepy, crawly critters because of a quote from later in her life: â€Å"No, we have been as usual asking the wrong question. I really do think that she has more potential being a woman president that will serve as the ultimate caring and nurturing model for the whole nation instead of having a stiff and rigid type of governance. One very basic attribute of Clinton which I very much like is the fact that she is a woman. In my observation, women leaders tend to be more compassionate towards the members of their jurisdiction. They are more open to suggestions of the general public therefore making them more transparent in understanding the true sentiment of the population. Just like a nurturing and caring mother, her stand about education is very evident that she is for the crucial benefit of the children which are seen as the basic foundation of the society’s future. With her plan to recruit more competitive teachers and increasing their salaries, she will build a connection towards how the right plan of governance should be implemented to affect even the youngest population of her generation. Although Obama, her crucial competitor, also have a good sentiment about public education, her approach towards targeting a surplus for teachers would be more effective instead of making public education more available without any promise for quality as what Obama plans to initiate if elected. She is very much supportive of capital punishment which is seen to greatly affect the development of a stringent form of law order. For this reflection I will use Bill as a pseudonym name for my patient as The Nursing and Midwifery Council (2010) states that. This covers situations where information is disclosed directly to the nurse or midwife and also to information that the nurse or midwife obtains from others. For this assignment Gibbs Reflective Cycle (1988) will be utilised as I feel comfortable with this model. Gibbs (1998) refers to the experience as an incident which involves exploring good and bad feelings, however Johns (1995) suggests that the fundamental purpose of reflective practice is to enable the practitioner to interpret an experience in order to learn from it. Type 1 diabetes is a lifelong condition in which the body cannot control the amount of glucose in the blood. This is because the body cannot produce the natural hormone insulin. Diabetes is a common, lifelong condition and as Zimmet et al (2001) identify that in developed countries one person in thirty may be affected and it is likely that by 2025 there will be three hundred million people with diabetes worldwide, this is mainly the result of more sedentary lifestyles and increased obesity. In 2006 Diabetes UK estimated that there were more than two million people with diagnosed diabetes and up to one million who are still undiagnosed (Diabetes UK 2006). A holistic approach to this long term condition is essential as it can come with so many complications. Complications may arise from inadequate management and treatment of the condition, which can adversely affect the quality of life and have financial implications for patients and the National Health Service (DH2001). There are two types of diabetes. In type 1 diabetes there is no production of insulin by the beta (ß) cells of the pancreas. In type 2 diabetes, which accounts for over 80 per cent of all cases of diabetes, insulin is produced by the ß cells and is released into the bloodstream, but it subsequently fails to act properly at the sites of glucose uptake, which are skeletal muscle, liver and adipose tissue (Donnelley and Garber 1999, Reginato and Lazar 1999). As a trainee assistant practitioner I was asked by the district sister to visit Bill to do a blood test which had been requested by the general practitioner as Bill is housebound and unable to attend a blood clinic. The blood test was to check his full blood count and HbA1c which had not been done for almost a year. Bill is seventy four years of age, lives alone and has lived with diabetes for many years. Bills wife passed away a year ago and has one son who lives many miles away so sees him very little; he does however have a neighbour who pops in to check on him now and again. Bill administers his own insulin in the mornings and checks his blood sugar levels daily before giving his insulin. My first impressions of Bill were that he looked frail and quite pale but having not met him before this may have been the norm for him. Whilst taking the blood sample I began chatting to Bill and he started to tell me that he had several episodes of feeling unwell recently and on that morning he had felt particularly unwell. I asked him to explain why he felt unwell and what symptoms he was experiencing. He explained the symptoms included shaking in his hands, feeling lightheaded and a fuzzy headache. As a trainee assistant practitioner I felt it necessary to explore what was wrong with Bill even further. First of all I began taking some basic clinical observations, his blood pressure was 140/90, pulse 80 and regular which were both within normal limits. He appeared pale and clammy so I checked his blood glucose level which was 3.2mmols; Bill was suffering from hypoglycaemia. Blood glucose levels are normally maintained within relatively narrow limits at about 5-7mmol/l (Williams an d Pickup 2004). My immediate concern was to ensure Bills blood glucose levels did not drop any further and the priority was to take short term action and increase his blood sugar to prevent it becoming any worse. Bill had no glucose tablets or glucogen so with his consent I looked in his fridge and cupboards to find something that would increase his blood sugars quickly. All that was in his fridge was a carton of milk a few slices of bread and some jam, I promptly gave him a drink of milk and made a jam sandwich. I felt it was my responsibility to sit with Bill until his blood glucose returned to acceptable levels and he had recovered from this episode of hypoglycaemia. I took Bills blood glucose levels every ten minutes until it returned to a safe and acceptable level. Bills blood sugar was now 5.2mmols and he was feeling brighter I checked to see if he ever recorded his blood glucose levels or kept a record of administration of his insulin but there was nothing. I asked him about his diet he said he hadnt been feeling up to eating much, I asked who did his shopping which he informed me his neighbour gets his milk and bread and a few other little bits when he needed them. I was aware that the lack of food in the house was probably the cause of Bill suffering from hypoglycaemic attacks. Hypoglycaemia occurs when the blood glucose level falls below 4mmol/L and is a common side effect of insulin therapy. Causes of hypoglycaemia include missed or late meals, not eating enough, taking too much insulin, exercise and excessive alcohol. National Health Services Choices (2009) state that hypoglycaemia should be treated with fast-acting carbohydrate, for example, 3-6 glucose tablets, 150ml fizzy drink or 50-100ml Lucozade, and followed up with a longer-acting carbohydrate, for example, biscuits or a sandwich. Glucose gels, for example, GlucoGel are useful to raise blood glucose levels and blood glucose should be recorded five to ten minutes after treatment. After ensuring that Bills hypoglycaemic attack had subsided and he was feeling better I made him a cup of tea and left him another sandwich that he could have at lunchtime. My initial feelings were of concern for Bills safety in the future and as a trainee assistant practitioner I knew that it was my responsibility to see my mentor immediately to discuss the situation. I was satisfied that I had taken the time to find out what was wrong with Bill and that he had recovered from his hypoglycaemic attack which I may not have taken time to do in my previous role. From the years of working in the community nursing setting experience I was fully aware that other mutli-displinary agencies may need to be involved in the care of Bill. I returned to the office and fed back to my mentor and later that day we returned to Bill and a full assessment was undertaken, it came to light that Bill had been struggling for some time with his diabetes, personal care and shopping and housework. It was decid ed by my mentor that for the interim period until care and support for Bill could be implemented that the district nursing team would administer his insulin that way his blood glucose levels could be regularly recorded and ensure that he has eaten something. He was also referred to the community diabetic nurse for a review of his insulin regime. Dietary management of type 1 and type 2 diabetes Nutritional therapy is an integral part of effective management of diabetes and has a vital role in helping people with diabetes to achieve and maintain optimal glycaemia control (Delahunt 1998, UKPDS 1990).I visited the general practitioner surgery and obtained some patient information on diabetes care and diet and took them to Bill, with the supervision of my mentor I sat with him and read through them. Once a care package was in place the carers would be informed of what foods Bill should and should not have and they would help with meal preparation. The British Diabetic Association (1999) suggest that ideally dietary information should be delivered by a diabetes specialist dietician, however in the case of Bill awaiting an appointment to see the dietician would have taken time and the information was needed on a more urgent basis. McGough (2003) suggest that structured patient education plays an important role in enabling people with diabetes to manage their diabetes on a day-to-day basis and a greater emphasis should be on the benefits of regular physical activity and weight management. More flexibility in the proportion of monounsaturated fat and carbohydrate in dietary intake and sucrose should no longer be restricted to a specific amount. For Bill initially it was essential that he was provided with regular meals and snacks at least three times daily to prevent any further hypoglycaemic attacks. An urgent referral was sent to members of the multi-disciplinary team and a meeting was arranged the following day with a social services. Referrals were also sent to the community diabetic nurse, dietician and foot health services. On assessment with my mentor she identified that Bill had not been washing properly and had not cut his toe nails for some time, Bill was also experiencing pain in his legs and feet. Bi ll was likely to be experiencing diabetic peripheral neuropathy, and I completed a pain assessment chart with him. Hill (2009) identifies that painful neuropathy affects the feet, typically causing burning or stabbing pain, which is particularly apparent at night. This was a mirror of what Bill described his pain as and we reassured him that his pain control would be discussed with his general practitioner as at present Bill took no analgesia at all and there was none in the house. The general practitioner prescribed paracetamol 1000mg four times daily initially as he felt that the pain may improve once more control had been gained again with his diabetic control. I returned to assess Bills pain control several days after commencing paracetamol and it had improved, he was still experiencing slight discomfort but felt that he would like to continue on this regime as he did not want anything stronger at the present time. It was agreed with Bill that this would be reviewed again the fo llowing week. A joint visit was done with the diabetic nurse, my mentor and myself and it was identified that Bills technique of giving his own insulin was poor due to poor dexterity in his hands and he was unable to turn his insulin pen properly or read the digits on the pen clearly. It was unclear how long Bill had been trying to manage in this way but Bill would certainly need long term care with his insulin from the district nursing team. The diabetic nurse identified that Bills eyesight was particularly poor and that he had not had his eyes checked for several years. Diabetic retinopathy is a major cause of blindness and many patients do not have any symptoms of the damage occurring in the retina until the complications have become advanced. NICE (2008) recommend annual screening for all patients with diabetes and that a record of the retina is made by digital imaging for year on year comparison to identify the development and progression of retinopathy. The general practitioner was informed that Bill had not had his eyes checked and he agreed that he would refer him for retinopathy screening. As a trainee assistant practitioner I have learnt valuable knowledge in the management and care of patients with diabetes, from Bill requiring a routine blood test he has become a complex patient with multiple problems related to his diabetes. Due to the word limitations of the essay all areas of complications relating to diabetes could not be covered but through researching and reading around the topic I am aware of other complications such as nephropathy, cardiovascular, cerebrovascular and peripheral vascular disease. The Earned Income Tax Credit (EITC) provides a tax credit for those who qualify with positive earnings, low income, and minimal capital gains. It is more so directed at working families, whether it is 2 parents or a single mother. It is intended to push more people into the work force, especially low-income women. In fiscal year 1998 the EITC is expected to cost the federal government $24.5 billion, $7 billion of which will result from expansions incorporated in the 1993 Omnibus Budget Reconciliation Act (OBRA93) (Houser Scholz, 2). It pushes for a â€Å"make-work pay† strategy of welfare reform so people do not rely as heavily on welfare programs. There are many articles that discuss what effect the Earned Income Tax Credit has on labor supply in America and the way the policy has affected so many. In â€Å"The Impact of the Earned Income Tax Credit and Social Policy Reforms on Work, Marriage, and Living Arrangements† by Ellwood, he examines how changes in social reforms, such as the EITC, have impacted labor supply, marriage, and cohabitation. Ellwood acquired and used the current population survey from March 1995 of women ages 18-44 who worked at least 26 weeks, also separating between married or single, with children. Then he used a wage equation to predict a potential 1998 wage for the women. Then the predicted wage was used to place the women into predicted wage/skill quartiles after accounting for education level. The incentives were much higher for low skill single mothers to work. Differentiating the women between skill and wage levels is critical in determining which group of people receives benefits and which don’t get affected at all. In 1986, she could hope to earn just $2,800 more and her effective tax rate being 76 percent; but by 1998, the number ha d jumped to $7,600 with her effective tax rate falling to 31 percent (Ellwood). The lowest quartile showed the greatest amount of change, and the following quartiles showed change, but nothing too drastic. Employment rates were rising for unmarried mothers. For married women, incentives to work were sharply reduced for low-income women, with effective tax rates falling slightly for women in other quartiles. A large change in work by the lowest quartile occurred with employment rates shooting up from 34 percent in 1992 to 55 percent in 1999. In regard to married women, some cases led to them being discouraged to work. Roughly 54 percent of married women in the bottom quartile would have faced penalties up to $1,288 (had they been earning $10,000) whose husbands likely were earning less than the EITC maximum. Another 28 percent would have had no incentive to work because their husband’s income surpassed the EITC maximum. The EITC encouraged more work force participation by sing le mothers, but hadn’t had such an effect on married mothers. Overall, based on the number of mothers in each group affected, this article concluded the EITC results in a net increase of working women. In â€Å"The Earned Income Tax Credit and Transfer Programs: A study of Labor Market and Program Participation† by Dickert and Scholz, they focus on the degree to which welfare policies, like the EITC, that alter after-tax wages affect hours of work, labor market participation, and welfare program participation. They used the following empirical model to study the effect of wages, taxes, and program benefits on labor market and program participation; bivariate probit models of labor market and transfer program participation. They included variables for income, transfers, and demographic characteristics in both participation equations. Two models are estimated separately: one for one-parent families and the other for primary earners in two-parents families. Net wages are key in influencing participation when it comes to the EITC. The empirical results for the single parent model found that net wages positively affected labor market participation. A 10 percent increase in the af ter-tax wage raises the single parent’s probability of working by two percent (Dickert Scholz). Two-parent families do not show any significant changes in labor market decisions for primary earners. Tax rates were also found to exert a strong negative effect on the probability of labor force participation. For single parents, the probability of working generally increases with predicted wage rates. In the bivariate probit model for single-parents families, a highly significant negative correlation is found between the labor market and transfer program participation. The article also examines the offset between the new participants in the labor force and the decrease in hours worked by those already working. The simulations show the EITC increases net wage of single parents by 15 percent, which in turn increases their probability of working by 3.3 percentage points. Assuming they work on average 20 hours a week for 20 weeks a year, this implies hours worked by single-parent f amilies would increase by 72.8 million hour per year (Dickert Scholz). Overall, the simulation results imply that greater labor market participation will lead to an increase of 74.4 million hours, compared to the 54.5 million hours less worked by those already in the work force, more than enough to offset. This article accounts for both the positive and negative effects EITC has on the labor force, and explains how the two cancel each other out with extra benefits to society. In the article â€Å"Welfare, The EITC, and the Labor Supply of Single Mothers† by Meyer and Rosenbaum, they focus on the effect that tax and transfer program had on incentivizing single mothers to work. They use the Current Population Survey Outgoing Rotation Group Files and the March CPS files. Estimates from these specifications suggest that EITC and other tax changes account for 60 percent of the 1984-1996 increase in employment of single mothers (relative to single women without children). Also, a 10 percent cut in the maximum benefit increases both annual and weekly employment rate by 1 percentage point (Meyer Rosenbaum). The period between 1984 and 1996 where tax and transfer policy was being pushed encouraged single mothers to work more. The one flaw I can find in this article is no discussion about the different phases that EITC entails. An individual experiences different incentives at different points of the EITC, and that is something that must be accounted for. A matter in question I believe needs changing is a way to figure out how to get change the fact that two-thirds of recipients, and 84 percent of total earnings, are in the phase-out range of EITC (Browning). It must be more focused on the phase-in region, where the growth of labor supply is actually occurring. The favorable benefits and results happen during the phase-in region, but sadly this is a small proportion of the population of recipients. A policy recommendation I would suggest would be to somewhat change the structure of the EITC to prevent this waste of money from transpiring. Perhaps a brand new welfare program needs to be put into place instead of the EITC that focuses more on the increased net wages for those who truly need it. An possible option would be for a recipient entering the phase-out region to report their earnings so that it may be determined whether they still should qualify to receive these net wage increases. In conclusion, these articles all feed off of one another to build up knowledge of the exact effects the EITC has on society. Single mothers experience higher incentives to join the work force, along with other low-income individuals. When structuring a model to research the effect of policy, it is crucial to take every variable into account. This finding was not surprising. CH3Br still accounts for roughly 50% of the organic bromine reaching the stratosphere. Determination of the ODP for methyl bromide primarily depends on the understanding of its total lifetime in the atmosphere, which in turn depends on the understanding of its re moval processes from the atmosphere. During the 1990s, it was discovered that ocean and soil sinks for methyl bromide need to be considered in determining the lifetime of methyl bromide. Those sinks are now considered in our analyses (a recent reevaluation of the ocean sink gave results very similar to the analyses used in the 1998 assessment). While there remains some uncertainty in the soil sink, this is unlikely to further change the ODP for methyl bromide in any significant manner. With ocean and soil removal processes now included in the analyses of its ODP, it is unlikely that there will be major changes in the ODP for methyl bromide unless there are significant changes to the understanding of its removal processes. The current evaluated global lifetime for CH3Br is 0.7 years, resulting in an ODP of 0.38. If all of the uncertainties in the soil sinks were to go the maximum amount towards increasing the sink, the ODP for CH3Br could decrease a little more. However, this is extr emely unlikely, as it would produce a huge inconsistency between the sources and sinks, exacerbating the difference already existing in the known sources and sinks. The bottom line is that the ODP for methyl bromide is unlikely to change further. As chlorine in the stratosphere decreases under the effects of the Montreal Protocol, the fact that stratospheric bromine is currently not decreasing means that bromine will become more and more important in its effects on ozone.
